If this book had been edited by Feigl rather than as a Festschrift for him it might well pass as Minnesota Studies in the Philosophy of Science, IV. There are twenty-six essays by as many contributors, most of whom are familiar faces in collections of this sort: Carnap, Scriven, Sellars, Meehl, Smart, Hanson, Popper, Salmon, Mehlberg, and the editors, to mention a few. Whenever the essays do occupy themselves with a discussion of Feigl's work, it is his work on the (...) mind-body problem and probability and induction that come in for the most extensive treatment. Of the essays taking off from Feigl's "The 'Mental' and the 'Physical,'" Meehl's "The Compleat Autocerebroscopist" is the best, being an exhaustive and often valuable re-consideration of some of the difficulties surrounding the identity theory of the mind-body problem. There is a good deal of less productive re-hash in the other essays, and one wonders whether the creative spurt in the philosophy of science in this country which owes as much to Feigl as anyone has started to wane.—E A. Background: Ethics support services are growing in Europe to help doctors in dealing with ethical difficulties. Currently, insufficient attention has been focused on the experiences of doctors who have faced ethical difficulties in these countries to provide an evidence base for the development of these services.Methods: A survey instrument was adapted to explore the types of ethical dilemma faced by European doctors, how they ranked the difficulty of these dilemmas, their satisfaction with the resolution of a recent ethically difficult case (...) and the types of help they would consider useful. The questionnaire was translated and given to general internists in Norway, Switzerland, Italy and the UK.Results: Survey respondents ranged in age from 28 to 82 years, and averaged 25 years in practice. Only a minority reported having access to ethics consultation in individual cases. The ethical difficulties most often reported as being encountered were uncertain or impaired decision-making capacity , disagreement among caregivers and limitation of treatment at the end of life . The frequency of most ethical difficulties varied among countries, as did the type of issue considered most difficult. The types of help most often identified as potentially useful were professional reassurance about the decision being correct , someone capable of providing specific advice , help in weighing outcomes and clarification of the issues . Few of the types of help expected to be useful varied among countries.Conclusion: Cultural differences may indeed influence how doctors perceive ethical difficulties. The type of help needed, however, did not vary markedly. The general structure of ethics support services would not have to be radically altered to suit cultural variations among the surveyed countries. (shrink)

Suicide is amongst the top ten causes of death for all age groups in most countries of the world. It is the second most important cause of death in the younger age group (15-19 yrs.) , second only to vehicular accidents. Attempted suicides are ten times the successful suicide figures, and 1-2% attempted suicides become successful suicides every year. Male sex, widowhood, single or divorced marital status, addiction to alcohol ordrugs, concomitant chronic physical or mental illness, past suicidal attempt, adverse (...) life events, staying in lodging homes or staying alone, or in areas with a changing population, all these conditions predispose people to suicides. The key factor probably is social isolation. An important WHO Study established that out of a total of 6003 suicides, 98% had a psychiatric disorder. Hence mental health professionals have an important role to play in the prevention and management of suicide. Moreover, social disintegration also increases suicides, as was witnessed in the Baltic States following collapse of the Soviet Union. Hence, reducing social isolation, preventing social disintegration and treating mental disorders is the three pronged attack that must be the crux of any public health programme to reduce/prevent suicide. This requires an integrated effort on the part of mental health professionals (including crisis intervention and medication/psychotherapy), governmental measures to tackle poverty and unemployment, and social attempts to reorient value systems and prevent sudden disintegration of norms and mores. Suicide prevention and control is thus a movement which involves the state, professionals, NGOs, volunteers and an enlightened public. Further, the Global Burden of Diseases Study has projected a rise of more than 50% in mental disorders by the year 2020 (from 9.7% in 1990to 15% in 2020). And one third of this rise will be due to Major Depression. One of the prominent causes of preventable mortality is suicidal attempts made by patients of Major Depression. Therefore facilities to tackle this condition need to be set up globally on a warfooting by governments, NGOs and health care delivery systems, if morbidity and mortality of the world population has to be seriously controlled . The need, first of all, is to identify suicide prevention as public health policy, just as we think in terms of Malaria or Polio eradication, or have achieved smallpox eradication. (shrink)

The Foreword and Epilogue are by two distinguished Indian educators, _K. L. Shrimali_, Vice-chancellor, and _N. A. Nikam_, former Vice-chancellor, University of Mysore.

The article is dedicated to the famous folklorist, literary critic, ethnographer, candidate of philological science, and doctor of historical science, Lev Grigorievich Barag, whose research and teaching activity for several decades was linked to the Bashkir State University. The authors of the article present main milestones of his scientific work as well as brief annotated overview of the major works of this outstanding Russian philologist in fairytale folklore and mark his contribution to the study of one of the most important (...) pages in the Russian literature of the 18th century, D. I. Fonvizin’s comediography. Multifaceted activities of Professor Barag are described, who successfully combined practical work of collecting folklore with research that was focused on the comparative study of fairytale epos. The authors consider the question of what place in Russian philology took this researcher, who made significant additions, refinements in the analysis of not only Eastern Slavic, but Turkic fairy story repertoire and who became one of the creators of the most authoritative index of East Slavic fairy stories and laid the foundations for the development of the study of Bashkir folklore. The main features of the Barag’s polemical research strategy aimed at the justification of potentially prorealistic method of Fonvizin-playwright were identified. Attention is drawn to the postulated by Barag judgments that anticipated modern scientific ideas about the processes of genre decanonization, about semiotic mechanism of transformation of the ‘unfamiliar‘ to ‘familiar‘, about genre originality of ‘The Minor‘ as a socio-psychological comedy, about trust of the author of ‘The Brigadier-General‘ in evaluative point of view of ‘nonpositive‘ characters. Along with political, economic, ethical, rehabilitative and military dimensions, psychopathological sequelae of war and terrorism also deserve our attention. The terrorist attack on the World Trade Centre ( W.T.C.) in 2001 and the Gulf War of 1990-91 gave rise to a number of psychiatric disturbances in the population, both adult and children, mainly in the form of Post-traumatic Stress disorder (PTSD). Nearly 75,000 people suffered psychological problems in South Manhattan alone due to that one terrorist attack on the WTC in (...) New York and the Pentagon in Washington. In Gulf War I, more that 1,00,000 US veterans reported a number of health problems on returning from war, whose claims the concerned government has denied in more than 90% cases. Extensive and comprehensive neurological damage to the brain of Gulf War I veterans has been reported by one study, as has damage to the basal ganglia in another, and Amyotrophic Lateral Sclerosis (ALS) in a third, possibly due to genetic mutations induced by exposure to biological and chemical agents, fumes from burning oil wells, landfills, mustard or other nerve gases. The recent Gulf War will no doubt give rise its own crop of PTSD and related disorders. In a cost-benefit analysis of the post Gulf War II scenario, the psychopathological effects of war and terrorism should become part of the social audit any civilized society engages in. Enlightened public opinion must become aware of the wider ramifications of war and terrorism so that appropriate action plans can be worked out. (shrink)

Most clinicians and researchers are concerned with recent advances in psychiatry. This involves the danger whether something time-tested may get sidelined for extra-scientific reasons. That the pharmaceutical industry and super-specialist researcher may keep churning out new findings to impress audiences is only a partial truth. Research progresses by refutation and self-correction. Acceptance in science is always provisional; changing paradigms, frameworks of enquiry and raising new questions is integral to break through in scientific knowledge. Hence, there is in science a constant (...) concern with the new. Moreover, the number of treatment non- responders to the time-tested swells with time, and researchers feel challenged to find ways and means of resolving their difficulties. Newer challenges need newer strategies. Obsession with the most recent can lead us astray, but a healthy evidence-based acceptance of the new is essential for advancement in psychiatric research. As indeed of research in all fields of medicine. And of science in general. The role of lithium and newer mood stabilizers in bipolar disorders are taken as examples to highlight this point. BACKGROUND: Reasoning biases have been suggested as risk factors for delusional ideation in both patients and non-clinical individuals. Still, it is unclear how these biases are longitudinally related to delusions in the general population. We hence aimed to investigate longitudinal associations between reasoning biases and delusional ideation in the general population. METHODS: We conducted an online cohort study with 1184 adults from the German and Swiss general population. Participants completed measures on reasoning biases (jumping-to-conclusion bias JTC, liberal acceptance bias LA, (...) bias against disconfirmatory evidence BADE, possibility of being mistaken PM) and delusional ideation at baseline, and delusional ideation 7 to 8 months later. RESULTS: A greater JTC bias was associated with a greater increase in delusional ideation over the following months. This association was better described by a positive quadratic relationship. Neither BADE, LA nor PM were associated with subsequent changes in delusional ideation. CONCLUSIONS: This study suggests that jumping-to-conclusions predicts delusional ideation in the general population but that this association may follow a quadratic trajectory. While no other associations turned significant, future studies with shorter temporal distances may shed further light on the role of reasoning biases as risk factors for delusional ideation in non-clinical samples. This paper examines the perceived ethical values of Malaysian managers. It is based on the opinions of 15 hypothetical ethical/unethical business situations from the 81 managers who agreed to participate in the survey. The findings of this study showed that these Malaysian managers have high ethical values. However 53% of the respondents believed that the ethical standards of today are lower than that of 15 years ago. Apparently, this is related to the existence of many unethical business practices prevalent in (...) the modern business world. The behavior of one's immediate superior is the most important factor in influencing managers to commit unethical practices. The results also indicate only a slight variation among the managers in terms of perceived ethical values by virtue of job position, job specialization, type of business activity or the size of the business organization. We prove that any such ideal is equation image, and conversely that if ℐ is any nonempty equation image ideal in the computably enumerable wtt-degrees then there is a computable enumerable real α such that ℐ = ℐ. Faced with the systemic persistence of immiseration, and given normative (...) morality's complicity in sustaining such systems of domination, what ethical and practical recourse do the oppressed have to liberate themselves? This is a question which runs throughout the book and motivates the problem of an 'insurrectionist ethics,' as coined by Leonard Harris: a field of ethics which takes up a position, as he writes in the foreword, "that is in, starts from, and sojourns to a different place" (p. xiii). It is a re-imagining of the ethical which departs from romanticized notions of agency and reason, often in ways that favor descriptions over normative or explanatory principles. This shift gives it a high degree of flexibility with respect to its approach, which nevertheless remains guided by a common "insurrectionist sentiment" that lends consistency to the works of the text's contributors: they all "favor the abolition of what creates non-being, living death and necro-being" (p. xi). The book is split into a total of five topical sections, each of which discusses a different facet of insurrectionist ethics: its conceptions and context, its ties to liberation theory throughout the Americas, applications and correctives for its theory, its relation to pragmatism, and its future. Bookending contributions from the editors unpack the meaning and definition of an insurrectionist ethics.In this review, I survey several chapters of the volume by discussing Scriven's and Carter's definitions and examining a few ways in which these definitions produce tensions in a few selected contributions. Doing so will highlight some important differences between [End Page 110] the authors' approaches to insurrectionist ethics while underscoring the consistency which commits them to abolition; ultimately, Insurrectionist Ethics is as much an invitation as it is an introduction to the tradition it espouses.Scriven's introductory chapter, "The Very Idea of Insurrectionist Ethics," exposes an insurrectionist ethic within the thought of David Walker, while Carter's conclusive essay, "Death by a Thousand Cuts: Insurrectionist Ethics in a Present less Oppressive than the Past," attempts to pin down a definition by analyzing its essential elements. Their difference in approach is not insignificant. Its status as a tradition means that insurrectionist ethics is not limited to the confines of this volume, itself more knot than archive: such a tradition reaches well beyond the text and invites its readers to extend its concepts and debates in all directions. Introduction: Consultation methods differ between medical practitioners depending on the individual setting. However, the central tenet to the doctor–patient relationship is the issue of confidentiality. This prospective survey highlights patient attitudes towards consultation methods in the setting of an ophthalmic outpatient department. Method: Questionnaires were completed by 100 consecutive patients, who had been seen by an ophthalmologist in a single room, which had a joint doctor–patient consultation occurring simultaneously. Results: Each question of all 100 questionnaires was completed. 58% of patients (...) were not concerned about sharing a consultation room with another patient or doctor. However, this did not equate to the 49% of patients who were indifferent to discussing issues in the joint consultation room. The most common factor was the general issue of confidentiality. Discussion: Ensuring total patient confidentiality may be deemed more necessary for certain medical specialties than for others, as seen in the practice of separate medical records in genitourinary medicine, for instance. However, with regard to patient consultations, the same level of confidentiality should be afforded across all specialties, and such factors should be borne in mind when planning outpatient clinics.
